The AIDS Support Organization (TAS0 Uganda), is an Organization contributing to quality health and social interventions with emphasis on HIV/ABS, TB, Malaria, and emerging public health threats through health and community systems strengthening, research, and capacity development in Uganda and other countries. TASO requires the services of dedicated, competent, innovative and result-oriented individuals for employment on a contract basis for the post below;
Job Title: (7) REF: TS/HR/PMGT/6/25: Community Linkages Assistant (02)
Classification: Contract full-time
Salary: Commensurate with training and experience
Reports to: Community Linkages Facilitator
Duty Station: TASO Jinja and Mbale CeE
Directly Supervises: None
Main Purpose of the Job:
To strengthen family and community support for PLHIV and their households by working closely with Community Linkages Facilitator (CLF), other peers, and community structures.
Key Outputs
• Community-based activities supported
• Community-facility continuum of care strengthened
• Linkages, referrals and networks conducted
• Communities updated on TASO services
Duties, Roles and Responsibilities
Participate in community mobilization and sensitization to create awareness about HIV/AIDS, health lifestyles and uptake of health services.
Participate in community dialogues/Community engagements on various health aspects
Actively document community activities conducted in the communities
Participate in writing monthly activity reports both community and facility activities
Conduct special emphasis on paediatric activities in the community.
Participate in conducting/creating Linkages and collaborate with other community health workers in the involvement of HIV/AIDS Prevention and Treatment programmes.
Work closely with the other health workers on client case management process to ensure that patients in need of specialist care are referred timely.
Help identify treatment failure cases and expedite their movement across the different levels of care at the facility
Conduct household hiv testing and ensure smooth linkage of positive cases for care
Conduct home visits for Paediatric, Adolescent and adults.
Implement various health services activities within the community: TES, TB ICCF, Strengthening of Social Support systems and strengthening STI, Preventions and all other medical related activities at the facility.
Conduct follow-ups (physical & telephone calls) at the clinic and in the communities for the unstable clients where necessary.
Ensure that all unstable clients are given quick and quality services at the centre and quickly moved back to treatment proximity (community).
Plan for Paediatric clients especially the children on treatment using viral load trends, adherence status and quickly move them to a nearer up-Family facility very close to their homes after regaining stability in treatment outcomes.
Qualifications, Education and Competencies
COMMUNITY LINKAGES SPECIFICATIONS
Minimum Academic Qualifications: Bachelor's in Social Work and Social Administration, Social Science, Education, Arts and Development, Development studies.
Additional Training Necessary
• HIV/AIDS Counselling Certificate
• Child and adolescent counselling
Minimum Work Experience: Practical experience of 2 years in HIV/AIDS support organization with competent knowledge of supporting client of both windowed, paediatric, adolescent and adult ages.
Special Knowledge: Counselling, Community Health and development, Knowledge in BCC model.
Key Skills: Analytical and problem-solving skills, communication and counselling skills, computer skills and organizing skills.
Other Qualities /Attributes: Integrity, Patience, Confidentiality, Self- motivated and a self-starter, able to work under mental and physical stress, Be able to speak the local languages of the duty stations.
