Step 1: Access the Form

Step 2: Complete the Form

• Type to fill in the form (handwritten forms will NOT be accepted).
• Print and sign the completed form.

Step 3: Prepare One Single PDF Document

Combine the following documents in this order into one PDF:

1. Copy of National ID
2. Passport-size photograph
3. Up-to-date Curriculum Vitae (CV)
4. Certified copies of academic certificates and transcripts

• UCE and UACE must be certified by UNEB

1. Names, contacts, and recommendation letters of:

• Two (2) Professional referees
• Two (2) Character referees

1. Certified copy of Declaration of Income, Assets and Liabilities (where applicable)
2. Copies of previous appointment, confirmation, and promotion letters (where applicable)
3. Any other relevant supporting documents

Step 4: Submit Online
