Job Description
African Hearts Community Organisation is seeking experienced and committed professionals to join its team in delivering impactful community development, social welfare, and organizational support services.
The Social Worker will provide social support services to vulnerable individuals, families, and communities. The role involves conducting assessments, developing intervention plans, facilitating access to services, advocating for beneficiaries, and supporting community development initiatives that improve wellbeing and social inclusion.
Duties, Roles and Responsibilities
- Conduct client assessments and identify social, economic, and psychosocial needs.
- Develop and implement case management and support plans.
- Provide counseling, guidance, and referrals to appropriate service providers.
- Support vulnerable children, youth, women, families, and other at-risk groups.
- Conduct community outreach, awareness, and sensitization activities.
- Collaborate with government agencies, community leaders, and partner organizations.
- Maintain accurate case files and client records.
- Prepare reports on program activities, beneficiary progress, and outcomes.
- Advocate for the rights and welfare of beneficiaries.
- Participate in monitoring and evaluation activities.
- Support community-based development and empowerment initiatives.
