Duties, Roles and Responsibilities
- Conduct comprehensive social assessments of patients' social, economic, family and environmental circumstances.
- Identify patients experiencing social vulnerability, neglect, abuse, discrimination or homelessness.
- Develop individualised social intervention plans based on assessed needs.
- Provide psychosocial support and counselling to patients and families.
- Advocate for patients' rights, dignity and access to social protection.
- Engage families and support discharge planning and community reintegration.
- Work with multidisciplinary healthcare teams and external stakeholders.
Qualifications, Education and Competencies
Qualifications & Experience:
- A0 in allied health science, social work or a related field.
- Experience in social work or psychosocial support is an advantage.
- Fluency in Kinyarwanda, English and/or French.
Skills & Competencies:
- Social assessment.
- Counselling and family engagement.
- Patient advocacy.
- Multidisciplinary teamwork.
